چکیده

Methods We study 23 children with CMA that met the following criteria: clinical history of IgE-mediated CMA in the last 6 months and specific IgE (sIgE) for cow’s milk (CM) ≥ 15 kU/L (> 2 years) or ≥ 5 kU/L (< 2 years). On study entry, basal serum levels for sIgE for CM and casein plus sIgG4 for casein were obtained. The study was divided into 2 stages. In the first 12 months, patients were randomly assigned in a double-blind manner to placebo or active OIT. At the end of the first 12 months period, patients submitted to milk double-blind placebocontrolled food challenge (DBPCFC). The study was unblinded in this time and patients on the placebo group started on a 24 months active OIT period. Children on active OIT completed 12 more months of treatment. sIgE for CM and casein and sIgG4 for casein were obtained throughout the study.

Clinical Predictors for Favorable Outcomes in an Oral Immunotherapy Program for IgE-Mediated Cow's Milk Allergy.

BACKGROUND Avoidance strategies in patients with cow's milk allergy occasionally fail to protect these patients from inadvertent exposures, leading to life-threatening reactions. OBJECTIVE To assess the safety and efficacy of milk oral immunotherapy as an alternative therapeutic strategy. METHODS Patients (n = 280, >4 years old) with IgE-mediated cow's milk allergy were enrolled into a milk...

Oral Food Desensitization in Children With IgE-Mediated Cow's Milk Allergy: Immunological Changes Underlying Desensitization

PURPOSE This study aimed to evaluate the safety and efficacy to induce clinical desensitization to cow's milk (CM) of an oral immunotherapy (OIT) protocol in a pediatric population with cow's milk allergy (CMA). In addition, the immune responses against β-casein, of peripheral blood mononuclear cells (PBMCs) from CMA patients, before and after the protocol were evaluated and compared to a nonal...

Safety and efficacy profile and immunological changes associated with oral immunotherapy for IgE-mediated cow's milk allergy in children: systematic review and meta-analysis.

BACKGROUND Cow's milk allergy (CMA), one of the main types of childhood allergy, considerably impairs patient quality of life. Allergen avoidance is difficult, and mistakes are common. Therefore, new treatment strategies such as oral immunotherapy (OIT) have been sought for patients with CMA. Our objective was to review current evidence on immunological changes, efficacy, and safety when using ...

حساسیت‌زدایی خوراکی در کودکان مبتلا به آلرژی به شیر گاو: کارآزمایی بالینی آینده‌نگر

Background: Cow's milk protein allergy (CMPA) is the most common food allergy during the first year of life. Strict avoidance of specific foods is the only accepted treatment for food-induced allergic reactions. This is often an unrealistic therapeutic option, since cow's milk is a basic food that is extensively used in infant formula. The recent preliminary experience of oral desensitization t...

Atopy patch tests are useful to predict oral tolerance in children with gastrointestinal symptoms related to non-IgE-mediated cow's milk allergy

Atopy patch tests (APTs) have been proposed for the diagnostic approach in children with non-IgE-mediated cow's milk allergy and gastrointestinal symptoms. We aimed to investigate the benefit of APTs in predicting oral tolerance in these patients. We prospectively evaluated 172 subjects with a sure diagnosis of non-IgE-mediated CMA and gastrointestinal symptoms (97 boys, 56.4%; age, 6.37 m; ran...
